Strengthening Oregon's Community Services (SOCS)

The SOCS Plan

Families, youth and service providers will join together to improve services for children and youth with special health needs.

SOCS Projects staff will:

  • Connect with community leaders to form a steering committee
  • Conduct a needs assessment of families
  • Organize a community forum of families, youth and providers that maps the strengths and service gaps of the community
  • Facilitate a partnership between families and providers to identify a community plan
  • Recruit and maintain a work group to ensure that the plan becomes a reality.

  • Empowers families to become leaders in promoting comprehensive quality care for Children with Special Health Needs (CSHN)
  • Strengthens family networking at the community level
  • Provides an opportunity for providers to hear from families of CSHN
  • Builds stronger partnerships between families and providers for CSHN

This is a HRSA funded grant and is being conducted in collaboration with the OHSU Oregon Rural Practice-based Research Network (ORPRN) and the Family Involvement Network (FIN).
