Organization of Services
How we address this outcome.
Multiple groups such as local inter-agency coordinating councils (LICCS), the CDRC, Oregon Department of Ed and Office of Family Health Inter-agency Team, and the Inclusive Child Care Committee convene regularly to work on community-based systems of care issues. The CaCoon and CCN programs engage local providers, schools, public health departments and a variety of key community partners in the care of CYSHN and the development of community systems to increase capacity to provide care and care coordination.
We are currently receiving technical assistance from Utah around the Universal Application System (UAS) called Utah Clicks. The Oregon Center has identified a group of agencies interested in developing a similar process in Oregon. The goal of the UAS is to create a web-based application process designed for families to be able to complete applications on-line for a variety of programs and resources. This system will screen families for services and make the process of completing paperwork much less redundant by providing families a one stop application that will direct them to appropriate service agencies, programs and more by filing out just one single from. This is a new and ambitious endeavour to make navigation of the system far easier for families than it has been in the past.
Key Systems Outcomes: Services for children with special health care needs and their families will be organized in ways that families can use them easily.
