Information for new patients

Welcome, and thank you for selecting the OHSU Comprehensive Pain Center. Listed below is important information for new patients to ensure your appointments with us run smoothly. Throughout the time you receive services from us, please feel welcome to contact our team with questions or need for information.

How to Become a Patient, and request your first appointment
To be seen at the CPC, you need to be referred to us from your current physician. Your physician's office needs to simply fill out a few forms and fax us copies of your chart notes. When we receive this information, we will call you to schedule your appointment. Please feel free to give your physician our phone number 503.494.PAIN (7246). The forms we need from your doctor can be found here

Before you come to your appointment
You will need to register before your first appointment by calling the OHSU Registration office at 503.494.8505. When we schedule your first appointment with you, we will transfer you to the Registration office so you may register at that time.If you have any changes in your insurance, please let us know; you will need to re-register with the Registration office.

What do I need on my first visit
Please bring to your first appointment your completed Questionnaire (we will mail this to you, or you may print it using the link above)

Do I need to bring copies of my medical records?
We need to obtain your medical records and your demographics from your doctor's office to understand your pain needs. Your doctor should send them to us at the time of your referral by submitting our required referral forms. Feel free to print out these forms and give them to your doctor. Before you come to your appointment, please check with your doctor's office to see if they have sent us your records, and if not, please obtain copies and bring them with you. If you have had x-rays or an MRI, you may be asked to bring these with you.

When should I arrive for my appointment?
Please arrive at the scheduled time of your appointment. This will ensure enough time for you to check-in with the reception desk and complete paperwork.

Late arrival for an appointment

Co-Payments
Co-payments must be paid upon arrival at your visit. We accept cash, check, and Visa/MasterCard and Discover Card.

Cancellations
We request that patients who are unable to keep an appointment contact our office at least 48-business hours prior to the scheduled appointment time, since there are usually other patients that could benefit from this treatment slot. 503.494.8544
