We encourage applications from anesthesiologists who will have completed their residency training before July 1st of the fellowship year they are applying for. We also accept applications from board certified or eligible neurologists and physiatrists interested in a pain management fellowship who have completed at least a one month Anesthesiology Airway elective and have had some sort of pain elective rotation.

If you would like to be considered as a candidate for our fellowship please download and complete our application and mail it to us in addition to the following information:
- Completed OHSU application form
- Current Curriculum Vitae
- Personal Statement
- Three letters of recommendation (one should be from your Residency Director)
- Medical School Dean's Letter and Official transcript (copies are sufficient for application but official copies will be needed if you are hired)
- Copies of USMLE scores steps 1, 2 & 3
- Verification of completed Internship & Residency
Please note that all letters of recommendation and verifications MUST be originals and on letterhead - Copies and generic letters are not accepted.
