Off Campus Authorization - Overview

Oregon Health & Science University (OHSU) clinicians, students, faculty, residents, fellows, and researchers who, as a part of their employment or clinical appointment with OHSU, are required to provide clinical services or research activities involving patients "off campus" (defined as any location that is neither owned nor leased by OHSU) must have express authorization in order for those services to be considered to be within the course and scope of their employment. As part of that authorization process, such clinicians are required to complete and submit the Off Campus Authorization Application form.
