Claims History - Overview

Claims History Reports for Oregon Health & Science University (OHSU) physicians are maintained by the OHSU Risk Management Department. All requests for Claims History Reports are processed and evaluated by risk analysts in a timely manner. Entities outside of OHSU that request Claims History Reports must pay a $40.00 non-refundable fee. Requests for Claims History Reports beyond five years are subject to an additional $40.00 non-refundable fee.

Any physician, hospital, or entity requesting a Claims History Report must provide a physician signed Authorization Form for Release of Records giving the requester permission to obtain a copy of a physician’s Claims History Report before Risk Management will release any information.
