FAQs - Certificate of Insurance

Q: How do we provide proof of self-insurance coverage to others?
A: Contact Risk Management if you are asked to provide a Certificate of Insurance to another party.

Q: What types of situations typically require a Certificate of Insurance?
A: Typical situations requiring a Certificate of Insurance include:

  • Many agreements will require a certificate of insurance as a term of the contract.
  • Certificates of Insurance are issued for official OHSU programs and activities.
  • On occasion, you may be asked to provide evidence of insurance for coverage for physical damage to property that is rented, leased or borrowed. The property must be deemed necessary to accomplish a mission critical use.
  • As a rule, OHSU does not name other entities as additional insured’s on our insurance policies.
  • Remember, liability insurance covers only the negligent acts of OHSU employees, agents and volunteers.

Q: What are examples of covered activities?
A: Examples of covered activities include:

  • Renting/leasing vehicles, heavy equipment and other equipment, for example copy machines
  • Leasing office space in a non-OHSU owned building
  • Equipment or vehicles loaned to an OHSU for a specific activity or purpose
  • Operating a booth at a trade show, conference or other special activity

Q: What are examples of situations that are not covered?
A: Examples of situations and/or people that are not covered:

  • Independent contractors
  • Student or employee events, unless they are part of a officially sanctioned event
  • Outside organizations i.e. OHSU Foundation
  • Non-OHSU activities
  • Use of equipment not related to an OHSU activity

Q: How do I request a Certificate of Insurance?
A: Follow these steps:

  • Review your proposed agreement carefully to identify areas where OSHU is assuming risk.
  • Work with Risk Management, the Legal Department, and Logistics to make certain proposed contract language follows OHSU standards.
  • If the agreement requires a Certificate of Insurance, contact Risk Management. You will be asked to complete the Certificate of Insurance Request form and fax it to Risk Management at least one week before you need the certificate issued. (A Certificate of Insurance is issued by Risk Management based upon the contract terms.)

Q: Are blanket or Generic Certificates of Insurance issued?
A: Except under limited circumstances, blanket or general certificates are not issued.
