Reconstructive Surgery

Required: -Referral letter from primary care physician.  Fax to 503-494-1717

Optional: -Copy of recent patient medical records that directly address the reason for the referral

  • Upon receipt of a referral letter, a member of our staff will contact the patient to schedule a consultation appointment

 

Cosmetic Surgery

         Required: -Nothing
