Additional Requirements

All Students

There are some additional requirements that must be fulfilled before you can begin your Visiting Student Elective Rotation.

1) Criminal Back Ground Check

Deadline: At least on month before start of the rotation

You must fill out and return this form. Be sure to include your correct SSN and your driver’s license or state id number and state of issue. Missing information will cause a delay in processing your application.

Please do not fax the form to Public Safety

even though it says so on the top of the form.

Follow instructions below for getting the form to OHSU.

The form needs to be signed off in our office before it goes to Public Safety. The pediatric education coordinator will fax it to them.  If you fax it to Public Safety yourself, they won’t know that the form is for a visiting student, and they won’t know what to do with it.

Criminal Background Check Form Link

Please fax or mail completed form to:
Pediatric Education Coordinator
Oregon Health & Science University
MC: CDRCP
3181 SW Sam Jackson Park Road
Portland, OR 97239
Fax:  503-494-4953

Please send this document as soon as your registration has been confirmed, this can be weeks or months before your rotation.

2)  LCME/MD Students using VSAS have two weeks to accept their placement using the VSAS system

3)  You must send in the $75 processing fee to complete your application. Students should receive an invoice from OHSU.

4)  You must send in a completed Immune Status Form.

5)  Please go to the visiting student website for details about these and any other requirements.

6) Housing

OHSU does not coordinate visiting student housing. Visiting students are responsible for their own housing. Some useful suggestions include:

Portland State University

They sometimes have some overflow housing available. They can be reached at www.pdx.edu.

Craigslist

http://www.portland.craigslist.org/ can also be a valuable resource.

OHSU

OHSU also provides some housing information at the following site.

http://www.ohsu.edu/academic/acad/housing/

7) Big Brain Pre-Arrival Requirements: Deadline before you arrive.

Big Brain is the OHSU web based training tool that you will use to complete two mandatory requirements.  Here is the link to "Big Brain" Log.

Big Brain

A. Complete HIPPA Privacy and You Training

B. Complete Respect at the University Training

Print both certificates and bring them with you, they are required to obtain an ID badge. YOU WILL NOT HAVE ACCESS TO A COMPUTER TO PRINT IT OUT WHEN YOU ARRIVE.

8) Review Course information and material:Deadline Before you arrive.

All students are encouraged to take some time to review the information and materials provided by the links on the right side of the page. Specific electives have contact information for lead preceptors, instructions for reporting on the student's first day, and any materials or handouts required for the course.

6) Parking and Transportation.

Parking Permit

A parking pass can be obtained at the information booth, the coast is $10. They take checks or cash, checks should be filled out before you get to the booth and made out to OHSU.  The information booth is the yellow question mark on your map near the corner of SW US Veterans Hospital Road and SW Sam Jackson Parkway.  They will ask which lot, the best lot is the “South Lot ” located on the corner of SW 6th Ave and SW Gaines. (see annotated map)

Parking Meter

On 6th Street just before it connects with SW Gaines Street there are long term 5 hour meters. The cost is $1 and hour and the machines take coins or debit/credit cards, no bills accepted.  You will need to make sure you add time before the time runs out.  Tickets are $24 for expired meters. (see annotated map)

Public Transit

Public transit is often the best way to get to OHSU. Please go to www.tri-met.org for Portland area transit information.

Review Pre-arrival Requirements

I have faxed a background check form to the Pediatric Education Coordinator at 503-494-4953. (Not the fax number on the form) and made sure my SSN and DL or State ID number are on it.

If I am an LCME/MD student I have accepted the appointment in VSAS .

Sent $75 processing Fee to Marcia Decaro at the OHSU Deans office as directed after receiving the invoice.

Sent Immune Status form to Marca DeCaro at the OHSU Dean's office.

I have completed all requirememts at this website.

Information and forms at Visiting Student Website

I have completed HIPPA in Big Brain and have a certificate of completion printed and ready to present on my first day.

I have completed Respect at the University in Big Brain and have a certificate of completion printed and ready to present on my first day.

I understand the OHSU doesn't provide scrubs.  If I am taking an elective with call,  Neonatology, Hematology/Oncology, Pediatric Intensive Care Unit, or the Pediatrics Sub-I, and I want scrubs to wear on call nights, I will need to bring them.

I have reviewed all materials associated with my elective by the first day.

I understand that parking difficult and have made transportation decisions.
