The pediatric selective rotation has been developed to allow for more individualized experiences to meet students varying career goals while continuing to provide an ambulatory experience to complement the inpatient Pediatrics 1 rotation. It is understood that each selective experience will be structured somewhat differently, but the overall goal is to further expose senior students to pediatric general and specialty patients and to allow those students to use this experience to more fully understand the field of pediatrics and pediatric surgical specialties, as well as to improve their ability to care for children.
The selective experience may consist of some combination of outpatient clinic, inpatient consult or service, operating room time, or time with various inpatient services (i.e., pain and sedation).

If you know of any site that you would like to set up your own rotation with that is not on this list, please follow the instructions for setting up your own rotation at the bottom of this document. You must work quickly if you want to set up a rotation. It may take time to get your preceptor a faculty appointment if he or she doesn’t already have one.
Subspecialty Selective Options
1. Developmental Pediatrics
The student will spend time in the evaluation and follow-up clinics of the Child Development and Rehabilitation Center. The focus is on child development and the spectrum of neurodevelopmental disabilities—mental retardation, learning disabilities, autism, cerebral palsy, spina bifida, and others. Interdisciplinary team evaluation and management planning are emphasized. This rotation is essentially the same as Pediatrics Elective 709 K Developmental Behavioral Pediatrics.
Availability: RARE. Only available to Pediatrics 2 students 2 or 3 rotations per year when no elective student is registered.

2. Developmental Pediatrics-Eugene
The student will spend time in the evaluation and follow-up clinics of the Child Development and Rehabilitation Center. Housing: Provided Housing is provided through Peace Health. It is an apartment shared with OHSU medical students in Eugene for Pediatrics and other clerkship rotations.

3. Pediatric Dermatology
This specialty has two primary attending, Dr. Krol and Dr. Siegel. Students will divide their time between Doernbecher Children's Hospital and the Center for Health and Healing. There are dermatology residents on this rotation. Students report a wonderful experience. Exposure to dermatology is seen as especially valuable to future pediatricians, family practice physicians, and internists. There are opportunities to go to care management rounds on Wednesday mornings and dermatology lectures. Please speak with your resident or attending about these additional learning opportunities.

4. Pediatric Hematology/Oncology
The student will spend time in the outpatient Hematology and Oncology Clinics at Doernbecher Children's Hospital. The rotation will focus on the care of pediatric hematology and oncology patients. There will be the opportunity for many procedures (lumbar punctures and bone marroe biopsies). The student will attend educational conferences in the pediatric hematology/oncology division. Each student is required to give a 20 minute oral presentation at the end of the rotation. While primarily an outpatient rotation, there is the opportunity to get some experience with inpatient hematology/oncology as well.
Previous students on this rotation report a lot of autonomy and great teachers. The nature of this clinical environment gives students the opportunity to see the same patients again and again; this continuity of care is a positive learning tool.

5. Pediatric Endocrinology
The student will spend time primarily in the metabolism clinic.Students may also explore other related pediatric subspecialties like genetics. Please talk with Dr. Hanna about these options. The student may participate in inpatient consultation should the need for this arise during the rotation. This rotation is essentially the same as the pediatric elective 709 H Pediatric Endocrinology.
Availability: SOMETIMES This site is willing to take student when there is not a third year student or elective student scheduled.

6. Pediatric Neurology
During this month, the student will become facile at the pediatric neurological examination and will be comfortable with basic concepts including anatomic localization of neurological deficits and normal developmental milestones. The student will see a variety of neurological problems, including epilepsy, headache, movement disorders such as cerebral palsy/spasticity and tic disorder, neuromuscular disorders, neurometabolic and genetic diseases. The student will spend time in the child neurology clinics at Doernbecher Children’s Hospital and Shriner's Outpatient Center, as well as with the inpatient consult service at Doernbecher. Additional time may be spent on observation of occupational and physical therapy, interpretation of EEG and neuro-imaging studies, and involvement with patients in the surgical epilepsy program who have been admitted for long-term EEG monitoring. Time will also be allowed for discussion and preparation of a project. This rotation is essentially the same as pediatric elective 709L Pediatric Neurology.

7. Pediatric Pulmonary
Students will be exposed to children with various lung diseases including cystic fibrosis, asthma, pneumonia, chronic cough, and aspiration. Students will attend 2 CF Clinics and 1 Pulmonary Clinic per week. They will round with the team on inpatients 3-4 days per week. They will attend various clinical conferences. There will be assigned reading materials and videotapes. Early in the rotation the student will choose a project and will present it to the pulmonary faculty at a division meeting. Only available to Pediatrics 2 students 2 or 3 rotations a year when no elective student is registered. This rotation is essentially the same as pediatric elective 709G Pediatric Pulmonology.

8. Pediatric Pain/Sedation
This specialty is divided between two days in sedation, two days with the pain management service, and one day in the operating room. Students have given positive feedback about this rotation, especially working with the pain management team. The lead preceptor/grader is Dr. Jeffrey Koh. Students have really enjoyed the exposure to procedures including intubation, venopuncture and line placement.

9. Pediatric Emergency Medicine
In this rotation, students will work 12 9-hour shifts during the month at the OHSU Pediatric Emergency Department. The majority of shifts will be with Drs. David Spiro and Garth Meckler; however certain hours and shifts will be covered by the other EM faculty. Students will be mentored by both residents and faculty. Students will be expected to perform the initial history and physical on their patients, as well as perform all pertinent procedures. Students will also attend didactic conferences, do required reading, and undergo an oral exam at the end of the rotation.

10. Pediatric Otolaryngology (ENT)
The medical student will spend their rotation with our pediatric otolaryngology, head and neck surgeons, Drs. Milczuk, MacArthur and Richardson. Time is divided between the clinic and the operating room. Typically, about half of the week is spent in the DCH outpatient Otolaryngology clinic and half of the week in the DCH operating rooms with our surgeons. Both experiences afford the student to hone their exam skills of the pediatric ear, nose, pharynx, larynx and neck. Procedures in the clinic that they will observe include: ear exam with otomicroscopy, flexible laryngoscopy, flexible nasopharyngoscopy/evaluation of velopharyngeal insufficiency. These exams are then reinforced by the ability to observe anatomy in the operating room during such procedures as: insertion of tympanostomy tubes with the microscope, nasal and sinus exams, neck exam, pharyngeal exam during tonsillectomy and adenoidectomy, and laryngeal/tracheal/bronchial exams during direct laryngoscopy and bronchoscopy.

11. Pediatric General Surgery
Dr. Mark Silen heads up this pediatric specialty option. This rotation is reported by students as a great experience for students interested in this specialty. Students scrub in on procedures from port placements to hernia repairs. This is a surgical rotation: rounds begin early.

12. Pediatric Neurosurgery
This rotation is led by Dr. Nathan Selden. This is a surgical rotation and highly motivated students, especially those interested in procedures, have thrived in the experience. It is a very specialized area and some students report participating less readily in the development of patient care plans due to the highly technical and complex nature of the specialty. Therefore, this clerkship can be more observational in nature then some of the other Pediatrics 2 options. This rotation offers a lot of time in the operating room with just one clinic day per week.

13. Pediatric Urology
The student is expected to participate in the Pediatric Urology clinics, surgery, and inpatient care. Attendance at Grand rounds and Saturday conference is expected. Patients with common pediatric conditions as UTI’s, enuresis, vesicoureteral reflux, hydronephrosis and genital anomalies will be seen and presented to the staff attending.
The student is expected to make a PowerPoint presentation on a subject of interest seen on the clinical rotation at Urology Grand Rounds on the last Monday of their rotation.

14. Shriner’s Orthopedics
This is a busy orthopedic specialty located at Shriner's Hospital on Marquam Hill. Students should be prepared for a rigorous experience with time in the operating room and in various clinics. The lead preceptor/grader is Dr. Michael Aiona. This rotation is largely self-directed and very flexible; students report they assist a great deal in the OR. Shriner's staff are open to having students in clinics and the OR. Students are encouraged to obtain a copy of the clinic and surgery schedule and tailor their experience. Keep in mind most of these clinics are follow up in nature. The highly specialized nature of this rotation may mean some time spent observing. Students have a chance to see both common orthopedic issues and more unusual conditions.

15. Kaiser Permanente Orthopedics
This is a specialty experience working with Kaiser's pediatric orthopedics department. Ron Turker, M.D. is the lead preceptor/grader. The student will also work with Stephen Renwick, M.D. There are opportunities for students to work in the OR, fracture clinic, and the outpatient clinic. Students will learn the basics of fracture management, splinting and casting. This rotation would be appropriate for a general education or a student considering orthopedics as a career. We hear Dr. Turker and Dr. Renwick are excellent teachers.

16. Allergy Associate Research Center
Dr. Michael Noonan is the grader and lead preceptor for this rotation. Students feel he has a strong commitment to teaching. There is quite a bit of autonomy; the student will need to be self motivated and willing to help define the experience with the preceptor. Students should know some of the patients are adults. The unique opportunity to combine patient care and clinical research exists for the interested student. Students are also exposed to procedures carried out in the office.

17. Outside/In Clinic/General Pediatrics Doernbecher/Mother Baby Unit-Doernbecher
Outside/In
This is a split rotation, requiring a flexible student. Monday and Wednesday mornings the student will work at the Outside/In Clinic. This clinic provides health care services for poor and homeless youth and young adults. The space is shared with a naturopathic clinic, needle exchange program, and many other services for the target population. The cut-off age for clinic access is 30. While many patients are adolescents, students will be required to work with many people in their thirties. Students report many of the patients are there for reproductive health, STI screening, and mental health issues. Residents, both internal medicine and pediatric, a nurse practitioner, and an OHSU adolescent health pediatrician make up the Outside/In team. Please keep in mind you will be working with a resident much of the time. Past students have noted residents on the rotation are also learners so sharing access to patients can be a negotiation. Students will likely work with residents when charting. Dr. Wayne Sells, OHSU faculty and Medical Director of the Outside/In Clinic, will provide a grade for the student. Additional teaching is provided by Jenny Gilhooly, PNP.
Doernbecher General Pediatrics Clinic
Tuesdays, Thursdays and Fridays as well as Monday and Wednesday afternoons the student will be working in the General Pediatrics Clinic at OHSU. Attendings and Senior residents share medical student teaching responsibilities in this clinic. There will also be interns and some residents participating in their continuity clinics on site. There are opportunities to work in pediatric specialty clinics, one day with ENT, two days in Child Development and Rehabilitation, and an afternoon in Nephrology. The student is graded by Dr. Carrie Phillipi who polls all interns, residents and attendings for feedback. She will combine her evaluation with Dr. Sells' evaluation to come up with a final grade for the student. Students report challenges when shifting gears from the adolescent and young adult population at Outside/In to the more typical pediatric populations at the Doernbecher Clinic. Students on this rotation should be comfortable with these transitions. Students report a high degree of autonomy as residents and attending are very familiar with medical student education at this level.

Physicians must have faculty appointments at OHSU in order to serve as preceptors for OHSU medical students. Check with the physicians. If they don’t have an appointment, you must submit their updated CV to pedstud@ohsu.
