Doernbecher Children’s Hospital offers a one-month clinical elective sub-internship in inpatient pediatrics.  This rotation is open to fourth-year students from OHSU and other institutions.  The clerkship allows the fourth year student to learn to care for complicated pediatric patients in the hospital based setting.  It provides the opportunity for students to demonstrate their competencies as part of a busy inpatient team and facilitates a substantive evaluation of students in an active and challenging hospital environment.

Goals and Objectives:

The core goal of this rotation is for students to develop skills that allow increasingly independent evaluation and management of hospitalized pediatric patients.  At the end of this rotation, students should have demonstrated competency in:

1.     Taking a thorough history and performing a complete physical exam

2.     Assessing a pediatric patient who requires inpatient care and formulating a plan of care

3.     Presenting a patient efficiently, focusing on the pertinent data

4.     Documenting accurately and appropriately in the electronic medical record

5.     Writing accurate and safe orders (to be cosigned by the resident)  in the electronic medical record

6.     Managing the logistics of patient care, including arranging diagnostic studies and discharge planning

7.     Performing common pediatric procedures

8.     Communicating well with patients, families, and the entire health care team and providing family centered care.

9.     Exhibiting professional and ethical behavior at all times, including following HIPPA guidelines, respecting confidentiality, and complying with hospital policies.

Expectations:

Students are expected to admit and manage patients on the pediatric inpatient service. Patient load is limited to 6 patients.

1.     Duration of the rotation is 4 weeks.

2.     Night call is approximately Q4, and will be taken with a pediatric senior resident and intern.  The call schedule will be determined by the student coordinator.

3.     Students will complete a history and physical exam and complete chart documentation for each patient they admit.

4.     Students will pre-round on their own patients daily and write a daily progress note.  The senior resident will see the patient as well, and cosign the student note.

5.     Students will present patients during bedside rounds in problem-based format, including the pertinent information and a detailed assessment and plan.

6.     Students will attend daily morning report and resident noon conferences.

7.     Students will provide at least one brief (15 min) teaching conference for the team.

Evaluations:

Students should expect regular and timely feedback from the senior resident and the ward attending throughout their rotation.  In addition, students will meet for a formal feedback session with the ward attending or the senior resident halfway through the rotation. Feedback should include areas of strength as well as areas and opportunities for improvement. Students should be prepared to provide feedback to the resident and attending as well, as their input is valuable and necessary for the continued improvement of the students’ experience on this clerkship. Students should arrange this meeting during their second week of rotation. At the completion of the clerkship, students will receive written evaluations from both attendings and senior residents.  Input from interns and other team members will be incorporated into these evaluations.

This course fulfills the fourth year sub internship requirement for OHSU Students.
