To apply online to OHSU School of Medicine Graduate Program, follow the steps below (you may want to print these directions for reference):

1. Go to the online application Web site (http://www.applyweb.com/apply/ohsumed)
2. Create your online application account.
3. Completely fill out the online application.
   Note: You can fill out the application in multiple sessions. Make sure to save the application each time you change or enter data. Then each time you wish to add information, you login using your same username and password to access your application. You can continue to change and update your application until you pay and submit your application to OHSU SOM Graduate Admissions.
4. A minimum of three references are required.
   
5. • Valid e-mail addresses are required for all references.
   • Please answer YES to the 'Waive Access' question in the reference section.
     
6. Before you pay and submit your completed application, make sure to PRINT and REVIEW the appropriate Check List an Signature Sheet:
7. • Masters Check List and Signature Sheet
   • PhD Check List and Signature Sheet
   • Note: Your online application will not be complete until the SOM Graduate Admissions office receives your "Check List and Signature Sheet" with the required supplemental materials.
8. Pay (by credit card or online check) and Submit your completed application.
9. • Your Reference request emails WILL NOT be sent until your application is submitted via CollegeNet.
     
   • Please submit the application to allow time for your references to respond PRIOR to the deadline listed for the Program and Degree.
     
10. Gather the required supplemental materials listed on your Masters or PhD Check List and Signature Sheet above.
    
11. Send all supplemental materials by the date noted on the Check List with your signed Masters or PhD Check List and Signature Sheet to:
