Benefits Summary

Summary of Benefits: ( From on campus, From off campus ** ). If you have medical benefits, you also have vision benefits through Vision Service Plan (VSP) and prescription benefits through PharmaCare.

Open Enrollment: Open enrollment occurs each October. Enrollment packets are mailed in September or early October by the Benefits Office. Choices you make by the end of October will go into effect January 1 of the following year. If you do not want to make changes to your medical or dental coverage, you do not have to do anything. However, it is advisable to look over the packet for rate changes and potential changes in plans offered. If your selected plan is discontinued and you do not make a selection, Human Resources will default your coverage to another plan. Please note: If you want to continue a Health Care Flexible Spending Account and/or a Dependent Care Flexible Spending Account, you must renew them annually.

Qualified Family Status Change: If you get married, have a child, or have a spouse whose insurance changes, you may be eligible to change your benefits outside of the open enrollment period.

Employee Health

The Employee Health office is located in Multnomah Pavilion room 1 SE. The phone number is 503-494-5271. You must have a tuberculosis (TB) screening every year through Employee Health. When you begin your training at OHSU, you are required to go to Employee Health to receive your TB screening. In subsequent years, you may receive your TB screening at the annual administrative reappointment meetings offered by the Office of Graduate Medical Education.




Employee ID Number

Your employee ID number is a unique identification number that is not your social security number. It is printed on your paycheck. If you do not have access to a paycheck or pay stub and need to be reminded of your employee ID number, please contact the GME office at 503-494-8652.




FMLA and OFLA

The Federal Family and Medical Leave Act (FMLA) and Oregon Family Leave Act (OFLA) are laws that protect your employment status while time is taken for maternity/paternity responsibilities or to care for a family member. FMLA provides 12 weeks of protected leave with benefits after 12 months of employment. OFLA provides 12 weeks of protected leave after 6 months of employment, however benefits are only covered until paid accruals are exhausted.

Beginning in the 2004/2005 academic year, house officers accrue 3 weeks of sick time for each year of employment at OHSU. Prior to 2004/2005, sick time was accrued at a rate of 2 weeks per year. House officers also currently receive 3 weeks of vacation for each year. Sick time rolls over at the end of each year, unlike vacation time, which must be used up before the end of the academic year. If either FMLA or OFLA is taken, sick time must be used before vacation time. Time taken without pay is also an option.

If possible, please complete all required paperwork and submit it to the GME office before taking leave.

After the birth or adoption of a child, please be sure to fill out a medical/dental update form to add your child to your insurance plan. You have 31 days from the birth or adoption to complete this form, otherwise you must wait until the next open enrollment period (every October, with an effective date of January 1) to make changes to your benefits. Forms are available from the GME office or on the O-zone at http://ozone.ohsu.edu/hr/docs/medical_dental_update_form.pdf .

Birth credit

OHSU GME residents and fellows who deliver their newborns at OHSU are eligible to receive a $450 credit. To receive this credit, please contact Debra Tomsen directly at tomsend@ohsu.edu or 503-494-5521.

Residents/fellows are required to complete the following online training programs:

HIPAA:
To complete the HIPAA training from an on-campus computer workstation, login at https://www.ohsu.edu/bigbrain/. From a remote computer, follow the instructions at http://www.ohsu.edu/cc/hipaa/login.shtml . You will need your employee ID number or Novell username and password.

Resident Family Network

The Resident Family Network is a group of partners of OHSU residents and fellows. The group has weekly play groups for families with children, monthly nights out, a book club and movie club, and seasonal parties and events.
