You have the option of changing your address directly by emailing address@ohsu.edu, but you must supply your employee ID number in the email. If you use this method, please let the Office of Graduate Medical Education and your program coordinator know of your new address.
The Office of Graduate Medical Education orders business cards at the start of your training and if you switch programs or change your name. If you need cards for any of these reasons, please feel free to contact the GME office at 503-494-8652 or gme@ohsu.edu . If you need to order business cards for any other reason, please contact your program coordinator.
A large number of resident call rooms are located in UHS 13C. Most call rooms are assigned for use by residents within specific programs, but there are also three unassigned call rooms, or "hotel rooms," open to all residents on a sign-up basis. Please report to the Office of Graduate Medical Education any issues regarding comfort or housekeeping. If you have any questions or concerns about specific call rooms or about the area in general, please feel free to contact the GME office at 503-494-8652 or gme@ohsu.edu .
Please contact the Office of Graduate Medical Education if you do not have your OHSU institutional Drug Enforcement Administration, or DEA, number. Please note: This DEA number is for use only at OHSU Hospital and OHSU clinics, including satellite clinics and the Veterans Affairs Medical Center, or VAMC. It is not legal to use your institutional DEA number at Legacy Portland Hospitals, Providence St. Vincent Hospital, or any other outside institution or clinic. OHSU patients may take the prescriptions written by residents/fellows using their institutional DEA numbers to any pharmacy they wish.
Because the institutional DEA number is not your personal DEA number, there is no DEA certificate. Also, please note that your DEA number is different from your medical license number.
For writing prescriptions you will also need the institutional NPI number. To obtain this, please contact the GME office at gme@ohsu.edu or 503-494-8652.
Please contact your program coordinator and/or program director for information regarding how to apply for an off-campus elective.
Please allow approximately 2 months of lead time for processing paperwork for electives, especially for sites outside of Oregon.
You will not have liability insurance coverage for any non-OHSU site if the proper paperwork is not completed and fully signed before you begin your elective. Furthermore, it may be financially detrimental to your program if paperwork isn't completed and fully signed. Please call the GME office at 503-494-8652 if you have any questions.
Electronic Medical Record Completion, or eMRC, allows clinicians to electronically view, edit, and sign dictated or scanned medical records from on-campus or remote computer workstations. Please contact Celine Davis at 503-494-8142 to set up an account and signature.
Financial planning may be a topic that you have not considered because you have been focused on your training as a physician. Meetings of the Graduate Medical Education Committee, or GMEC, are typically held on the fourth Thursday of every month (except for the months of November and December, when they are held on the third Thursday due to the holidays) from 12 noon to 1:30 p.m. in the Marquam Room on the first floor of Mackenzie Hall. This meeting is open to all residents and fellows, and the conversation is interesting and a place for your issues to be heard.

House Officers Association There is also a Hospital Residents Forum that meets regularly to discuss various issues ranging from the state of medical care in Oregon to OHSU's medical record systems.
OHSU publishes a photo roster of all residents and fellows for each academic year. It is distributed to the various departments so they can identify residents.
Each spring the Office of Graduate Medical Education posts general housing information as well as information regarding specific rentals or houses for sale to incoming residents and fellows on the Graduate Medical Education website. Click here to view 2008 listings.

Lost or Stolen ID Badges: If your employee ID badge is lost or stolen, please notify the Department of Public Safety at 503-494-7744 as soon as possible. Then go to the GME office in Mackenzie Hall room 4148 to fill out a form for a replacement ID badge and a replacement meal ticket, if applicable. There is a $5 charge for a new ID badge. The Office of Graduate Medical Education pays the $10 fee for replacing the access card.
Program Transfers: The Office of Graduate Medical Education provides forms for new ID badges at its annual administrative resident/fellow reappointment meetings in May. If you transfer from one program to another at any other time during the year, please go to the GME office in Mackenzie Hall room 4148 to fill out a form for a new badge and a replacement meal ticket, if applicable. Please let the GME office know when you plan to get your new ID so we can transfer your meal ticket monies from your old to your new meal ticket number.
If you have received a summons for jury duty and do not wish to or cannot serve, please contact the GME office as soon as you receive the notice. You are welcome to either fax the notice to 503-494-8513 or drop the notice off at the GME office in Mackenzie Hall room 4148. We will compose a letter requesting that you be excused from service. Please do not submit your own request to be excused. If you would like to serve on jury duty, please arrange your schedule with your program coordinator.
The GME office orders two lab coats for each resident/fellow at the start of their training and if they transfer from one program to another or change their name. The GME office will order additional lab coats for those residents/fellows who feel they need them. If you need lab coats, please contact the GME office at 503-494-8652 or gme@ohsu.edu . Orders will be submitted once a month from August 1 through April 1.

Limited Licenses: For information about limited medical licenses, go to http://egov.oregon.gov/BME/phyapphomepg.shtml For an application form, contact the GME office at 503-494-8652 or contact the Oregon Board of Medical Examiners directly at 503-229-5873.
Unlimited Licenses: For an application, go to http://egov.oregon.gov/BME/phyapphomepg.shtml. You MUST have an unlimited license in order to moonlight. You must have passed USMLE Step 3 within seven years of passing Step 1 to qualify for an unlimited license in Oregon. It takes about ten weeks from the time an unlimited license application is completed until the time it is issued. Be sure to allow ample time for this process if you are applying for July 1, when your limited license expires. There is no grace period and you must be licensed from the PGY 2 year onward. The application materials are due two months prior. All unlimited licenses expire every two years; the next expiration date is 12/31/2009.
Claims History If you need a form outlining your specific liability insurance coverage, including your claims history or other information, please contact the Risk Management Department at 503-494-7189.
You may also renew your ACLS certification by contacting Dana Fisher at OHSU (fisherda@ohsu.edu). You will receive a CD for self study, an ACLS manual, and a software license number. After you have completed the computer training, you must sign up for a MegaCode to complete the renewal process.

The Office of Graduate Medical Education will pay for all these courses in advance if you inform us. For training through Portland Community College, the GME office has course lists that include a CRN number that you can use to have the Office of Graduate Medical Education billed directly. For courses at OHSU, the Office of Graduate Medical Education will be billed directly.
Regardless of where you take your course, the textbook is available at the OHSU bookstore, and the bookstore will bill GME directly if you show your ID badge, proving you are a resident/fellow.

If you are working in an environment that needs attention (e.g., computers malfunctioning, temperature or lights in call rooms need adjusting, call rooms need cleaning, work environment is unsafe), please contact the Office of Graduate Medical Education at 503-494-8652 or gme@ohsu.edu so we can arrange for solutions.
Lost Meal Tickets: If you have lost your employee ID card and thus your meal ticket, please come to the GME office in Mackenzie Hall room 4148 for a replacement.
Meal Ticket Policies:
Each resident/fellow who has a schedule that requires staying at OHSU beyond the usual workday/night is provided with a meal ticket. We request that residents/fellows provide the first meal of their shifts, but subsequent meals are paid for by the Office of Graduate Medical Education.
If a resident/fellow works for a period of longer than 12 hours, the Office of Graduate Medical Education will provide monies for one meal ($7, usually for dinner). If a resident/fellow works a 24-hour call, the Office of Graduate Medical Education will provide monies for two meals ($12, usually for dinner and breakfast). If a resident/fellow works a 30-hour call, the Office of Graduate Medical Education will provide monies for three meals ($18).
The Office of Graduate Medical Education asks program coordinators to provide the average number of long days, 24-hour calls, and 30-hour calls worked by residents/fellows in their programs each month based on rotation. The Office of Graduate Medical Education assigns monies to each meal ticket every two months based upon these rotations.
Click here for a list of the amounts assigned to each program and rotation. Each time you use your meal ticket, you will receive a receipt that indicates how much money is left on your account. If you use your meal ticket as intended and are running out of funds, please contact the GME office at 503-494-8652 for a review of your account. Monies will be rolled over from month to month.
You may elect to have a percentage of your meal ticket monies put onto a gift card to be used at the Daily Cafe and Coffee Bar at the Center for Health and Healing (CHH). The percentage will be the same for the entire academic year. Because the cards are treated like cash, they are not replaceable if lost.
Restrictions: Please do not use your meal ticket funds to pay for meals for faculty, students, or other staff. Meal tickets are also not intended for purchasing products or for funding a shopping spree at the end of the year.
Locations: You may use your meal ticket at the 3rd floor OHSU Hospital cafeteria, Hatfield Café, CEI Café, OPC Café, Multnomah Pavilion Café, and at the coffee carts. The only places at OHSU that will not accept meal tickets are the MacHall Cafeteria, the BICC coffee cart, and the Starbucks in Doernbecher Children's Hospital.
GME residents and fellows are welcome to stop by the GME office in Mackenzie Hall room 4148 for free notary services. Be sure to bring a form of identification with you. A driver's license is preferred as a legal form of identification.
Residents/fellows are required to complete the following online training programs:
HIPAA: To complete the HIPAA training from an on-campus computer workstation, login at https://www.ohsu.edu/bigbrain/. From a remote computer, follow the instructions at http://www.ohsu.edu/cc/hipaa/login.shtml . You will need your employee ID number or Novell username and password.
Respect at University: Login at https://www.ohsu.edu/bigbrain/ . You will need your employee ID number or Novell username and password.
The Competent Physician: Login at https://www.ohsu.edu/bigbrain/ . You will need your employee ID number or Novell username and password.
If you are having a problem with your pager or need a temporary loaner pager, please contact Wireless Services at 503-494-9150 or wireless@ohsu.edu . Please note: If you do not return your loaner pager, you will be charged for the cost of replacing the pager. If you need a long-distance pager, please contact the GME office at 503-494-8652. GME will contact Wireless Services to authorize the pager and will let you know when it is ready to be picked up.
Below is the contact information for the Parking Office as well as the parking policy for residents/fellows. The Parking Office offers some benefits for residents/fellows.
First, those residents/fellows who don't purchase yearly or monthly parking passes but occasionally need to park on campus may purchase hang tags (daily, 4-hour, and 2-hour) from the GME office in Mackenzie Hall room 4148 or from their program coordinators. The advantage of these hang tags is that they allow you to arrive and park when the Parking Office is closed, so you are not forced to park without a pass. Be aware that the date written on the hang tag must be the current date and must be be clearly written. If the date on the hang tag appears to have been changed, your car could be booted.
Second, for those programs that have rotations requiring residents/fellows to arrive midday and depart in the middle of the night, there are parking spaces reserved in parking structure 2 on the 7th floor. Please use these spaces only if you have the duty hours described and are in the programs indicated on the sign for the space.
If you have received a parking ticket, you must pay it or appeal it using this form ( From on campus, From off campus ** ) within 30 days, otherwise the fine will double and you could get a boot on your car. If you have concerns about your interactions with the Parking Office, please contact the GME office at 503-494-8652.
Although the Office of Graduate Education is not well informed on patient care issues, please feel free to send us suggestions for useful information to post at this site.
In the spring, the Office of Graduate Medical Education holds approximately ten administrative reappointment sessions for returning residents and fellows. These meetings are designed to save residents and fellows time by enabling them to complete several reappointment procedures at once, including renewing their contracts, completing their annual tuberculosis screening, renewing or withdrawing their parking privileges, and completing and submitting their annual limited license applications. These sessions are required for all returning residents and fellows.

The Resident Library is located in UHS 13B24. The room contains six computers, a fax, scanners, phones, a small refrigerator, and lockers. Please contact the GME office at 503-494-8652 or gme@ohsu.edu if you would like to be assigned a locker. We ask that you not use your own lock on the lockers so we can reclaim them when you leave OHSU. If this room needs any maintenance, supplies, or upgraded computers or phones, please let the GME office know.
Your program coordinator will assist you in obtaining a bar code for scrubs. If you have misplaced your bar code, please contact Patsy at 503-494-2294.
Upon completion of your graduate medical education program, please report your new address to the Office of Graduate Medical Education so we know where to send your W-2 form the following January. Click here for more information on what the GME office needs from departing residents and fellows.
If you have an on-the-job injury, please go to the nearest Emergency Room immediately. Be sure to specify that you are an OHSU employee, that your reason for seeking care is due to an on-the-job injury, and that your treatment is covered by workers' compensation. After receiving treatment, please go to the webpage http://www.ohsu.edu/risk/workers.htm for information and to complete the necessary claims forms.
