Grand Rounds Details:Please join us for a virtual Grand Rounds presentation, "Cognitive Impairment: Efficient Evaluation and Strategies to Improve Care", presented by Barak Gaster, M.D. and Jaqueline Raetz, M.D. The prevalence of Alzheimer’s disease and related disorders is rising, and PCPs will increasingly play a role in its detection. Most PCPs lack an efficient framework for evaluating cognitive concerns. This presentation will provide an overview of the Cognition in Primary Care Program, including outcome data from its implementation in a network of 14 primary care clinics. The program is designed to be concise and easy to replicate in other health systems, without requiring new staff or infrastructure, with the goal of enhancing cognitive evaluation and improving care of patients with dementia.
