Printing Instructions: The default setting on OHSU's computers results in the form printing in a reduced size. Therefore, we ask that if you choose to print and duplicate these forms yourself, you please make the following adjustments to your Acrobat Reader print settings (accessed through File/Print).
- Select the check box next to "Expand small pages to paper size"
- Deselect "Shrink oversized pages..." and "Auto-rotate"
This adjustment only needs to be done once - the settings will remain until Acrobat Reader is updated. These settings are necessary for optimal printing.
