Let OHSU's Department of Patient Relations help you find answers and assistance when you have problems, complaints or concerns. At OHSU, we listen; we inquire; we respond.

OHSU strives to provide patients with the highest quality of health care in a safe and supportive environment. Obtaining care is very personal and often stressful. We realize that misunderstandings and inconveniences sometimes occur.

That's why we listen — and take action — if you have questions about the care you have received or the way you have been treated.

We encourage you, your family, or your representative to first discuss your issues with your health care team. The physicians, nurses and other health care professionals who have been involved in your care are most familiar with your particular situation. Usually they will be able to quickly resolve any issues you may have.

If your concerns require more time and attention, our staff in the Department of Patient Relations is ready to help. We pledge to devote the time and energy needed to address your concerns. Patient Relations helps patients, seeks solutions and finds resolutions.

You, your family member or representative have a right to have your problems heard and your complaints addressed. If a member of your health care team is unable to satisfy your concerns, or if you are not satisfied with the response you have received, contact the Department of Patient Relations.

It is important to us that you feel comfortable turning to Patient Relations for assistance. We will act impartially in helping you resolve your problems. OHSU's Patient Advocate is your ombudsman and is authorized to assist you in finding a resolution to your concerns.

The OHSU Process

We ask patients or families who have problems, complaints or concerns to follow the process described below. These steps ensure patients first seek assistance from staff members who are most familiar with their care, and who can provide additional options if needed:

1. Please speak with the members of your health care team first. Usually, they can quickly resolve most issues.

2. If you are not satisfied with the response you have received from your health care team, call Patient Relations at 503-494-7959 or stop by the office to talk with our staff or to request a complaint form. You may also send your complaint electronically by a variety of means, including the online complaint form linked below.

3. Please ask the Patient Relations staff for assistance if you need help filing your concern.
