Need To Request a Patient Nutritional Consult?
Enter consult in EPIC or Call the Nutrition Office At
4-8795

Need to Order a Late Tray?
Please use EPIC to change the patients Nutrition order if necessary for the request
(ie. the patient has changed from NPO to regular diet and now needs a tray or other similar changes).

You can go to the web at food/food.aspx to enter specific food requests (Please do not fax a request)

Need Floor Supplies?
Fill out the Par Stock Request and Fax to 4-3773 by 6:30am
