National POLST Paradigm Initiative Task Force Newsletter (PDF)July 29, 2008 POLST TrainingJuly 23, 2008 Respecting Choices®, an Advance Care Planning Service based in LaCrosse, WI, is offering a new training program that will improve the skill of health care professionals in facilitating discussions to help patients or their surrogates make informed decisions when completing a POLST paradigm form. This new curriculum includes both a POLST paradigm facilitator training and an instructor certification.
