A message from our Chair, Dr. Paul Flint:
Thank you for choosing the Department of Otolaryngology-Head & Neck Surgery at Oregon Health & Science University for your health care needs. Our faculty and staff are committed to your well-being and will work to provide you with the highest quality of care in a timely, compassionate manner.
In addition to patient care, our doctors are dedicated to teaching and improving the quality of health care through clinical and basic science research. Research in our department has already led to important breakthroughs in understanding and treating disorders of hearing and balance, sinus disease, facial plastics & rehabilitation, and head and neck cancer. To maintain and support these programs we must rely on many sources of funding, including gifts from grateful patients.
Over the next few months, we will be sending out a special form, called the "Authorization for Use of Information By the OHSU Foundation for Charitable Giving". This form authorizes us to not only send you very important information about our programs, research, and events, but also allows us to send you requests for support. Please take a moment to sign and return the form, and allow us to share with you upcoming opportunities to join us in making a real difference in the lives of our patients and all those who will one day benefit from our research efforts. On behalf of our entire department, we sincerely appreciate your thoughtful consideration. Exceptional care and life-changing advancements are only possible through your generosity and vision.
Again, we are grateful for your support and for choosing OHSU Otolaryngology / Head & Neck Surgery for your care.
Warm regards,
Paul W. Flint, MD

Support OHSU Otolaryngology
To fulfill our vital mission of healing, teaching, and discovery, Oregon Health & Science University relies on the generosity of individuals, businesses, foundations and other associations. For that reason, OHSU has established the OHSU Foundation - a 501(c)3 organization that raises funds for the University and invests and manages those funds in accordance with donors' wishes. Each year tens of thousands of donors contribute millions of dollars in financial support to OHSU through the OHSU Foundation, through outright or deferred gifts. Donors may designate their gifts to any OHSU school, research unit, program, project or initiative - or leave their gifts unrestricted to support an area of greatest need. The form Dr Flint spoke of in his message is only one way to support our efforts. You can also go online to the OHSU Foundation and complete the Foundation's "Make a Gift" form. The links are below for your convenience.

Another way to support our mission, without completing the online forms, is to complete the form "Authorization for Use of Information By the OHSU Foundation for Charitable Giving". This form, when completed and returned to us, gives us permission to have one of our Foundation representatives contact you about supporting our program. It also gives us permission to send you information about our programs. When you have completed the form, please send it to Department of Otolaryngology, Attn. Office of the Chair, 3181 SW Sam Jackson Park Road PV01, Portland, OR 97239. If you have any questions, you can contact the Office of Integrity at 503-494-0219 or the Department of Otolaryngology via email at enthns@ohsu.edu
