This link will take you to a short survey that has space for you to type your thoughts. If you prefer to complete the survey on the phone, or have a copy mailed to you, please call 503-494-8083.

Opt-Out Information

If you would like to request a bracelet to opt-out of the PRIMED cardiac resuscitation trial,
please call 503-494-8083 and leave your name and mailing address. Your address will not be used for any purpose other than to mail your bracelet and to send you a letter to notify you that the study has ended.

If you already have a bracelet from the Hypertonic Saline trauma trial, the same bracelet will work as an opt-out alert for the ROC PRIMED trial, as well as for any similar future trials in our community.
