Below is an online form for OHSU Residents to register for our ACLS courses. If you are not a resident, then you must either register online via credit card, or download our registration form and mail it to our department with account string and fiscal authority signature. If you are not a resident at OHSU or affiliated with OHSU please use the registration form and mail it with a check made payable to OHSU Department of Emergency Medicine.
