Fatality Reports

The Oregon Occupational Fatality Assessment and Control Evaluation (OR-FACE) group at CROET has developed the following reports and safety alerts related to Janitors and Custodians. Each report (in Adobe PDF format) contains the group’s findings of an incident that resulted in a worker’s death, along with recommendations for preventing similar accidents in the future.
