Family Support Program (FSP)

The Family Support Program provides financial assistance for products, resources and other service items related to the care of a child with special health needs.

General Eligibility

  • Children birth to 21 years of age
  • Families must live in Oregon or pay Oregon income tax

Medical Eligibility

Medical eligibility for includes children that have long-term developmental, physical, or medical conditions. 

All three of the following elements must exist for a child to be eligible for the Family Support Program.

  • The special health need is complex enough to: 
    • restrict growth, development, or
    • restrict the ability to engage in usual activities, or
    • create a functional limitation
  • The condition requires specialty medical  care or more medical services than a typically developing child of the same age.
  • The condition has lasted or is likely to persist at least 12 months

According to this definition a child with a diagnosed chronic, long-term special health need or disability would not be eligible unless they were currently receiving specialized services and were experiencing limitations as a result of that condition.

Financial Eligibility

Raising a child with a special health need can cause financial hardship for families, regardless of their income. Therefore, OCCYSHN will not impose an income limit or conduct a formal financial screening process to determine eligibility, however a demonstrated financial need must exist.  

Financial eligibility is based on the following:

  • Family financial need will be determined by the individual who is facilitating the request for FSP funds on behalf of the family. A statement of financial need should be included on the request form.          
  • Due to limited funding, priority is given to those families who have a true demonstrated need.
  • FSP dollars are the payment of last resort.
  • Annual cap of $750.00 per child, per year. 

How to Apply:

FSP applications can be submitted through a representative of one of the Title V Programs (CaCoon, Community Connections Network (CCN), Strengthening Oregon Community Services (SOCS) or Family Involvement Network (FIN)) or directly to the Family Support Program by a health, education, or agency advocate.
