Community-Based Programs

The Oregon Center for CYSHN   Community-based programs CaCoon and Community Connections Network (CCN)   provide for a statewide system of care for children and youth with special health needs through the sponsorship of multi disciplinary outreach clinics and support of public health nurses that provide and promote family-centered, care coordination that is culturally sensitive, and provided as close to home as possible.  We work with primary care offices to promote a Medical Home model.

We work to discover and collect resources from around the state which you can access through the county search of our statewide map.  We bring Training and Professional development opportunities to sites around the state.

Our engagement with communities takes on many different forms.  We are able to provide support to existing efforts through Program Evaluation.  We are able to provide financial resources to families in communities through our Family Support Program
