CCN TOOLBOX

Community Connections Network (CCN) is a network of community-based teams across Oregon. These multidisciplinary teams promote optimal health and development in children and youth with special health needs. CCN efforts are in keeping with the Maternal Child Health Bureau performance measures. Efforts include:

• Direct Services for CYSHN: CCN teams work with parents as professional partners to create plans for coordinated care.
• Community Trainings: CCN provides in-service trainings and consultations by specialists in areas of interest or need.
• Improved Systems of Care: CCN increases interagency communication and collaboration. Teams identify and address gaps or barriers to health and community services.

Children and youth with special health needs (CYSHN) are children who have or are at risk for chronic physical, developmental, behavioral or emotional conditions and who also require health and related services of a type of amount beyond that required by children generally. – Maternal Child Health Bureau
