How to apply for Social Security benefits
- Call 1 (800) 772-1213
- You can not apply for SSI online for childhood disabilities.
- Call the above number, give them your zip code, and they will make an appointment for you at your local office.
- Schedule the appointment and complete the paperwork.
- Additional assessment may be scheduled.
- Within 90-120 days a determination will be made.
How to apply for Early Intervention (EI) or Early Childhood Special Education (ECSE) services
- Call the intake number located on the following website: Oregon Department of Education (external link).
- Parents, educators, physicians, preschool teachers and childcare providers may refer a child.
- An assessment is completed.
- EI: For children 0-2 years of age
- Children's cognitive, adaptive, personal-social, motor and communication skills are assessed.
- ECSE: For children 3-Kindergarten
- Children are assessed in the areas where a developmental difference is suspect
- EI: For children 0-2 years of age
- Determination of eligibility and services.
- Educators, assessment personnel and care givers are involved in the eligibility decision.
- The evaluation is conducted over at least 2 days in at least 2 different locations.
- If the child is eligible for services, an Individual Family Service Plan (IFSP) is generated. This is a list of goals and services the child will receive to address their skill deficits.
- Completion Time:
- EI: Determination of eligibility will be completed within 45 days.
- ECSE: Determination of eligibility may take longer than 45 days.
How to apply for Regional Program Services
EI/ECSE programs will refer your child for regional services as needed.
How to apply for Respite Services
There are multiple respite care providers. Oregon Department of Human Servies Lifespan Respite Care can be accessed by going to http://www.dhs.state.or.us/seniors/caregiving/respite_care.htm (external link).
