HIPAA: Designated Record Set

HIPAA allows patients to request access to (and if they’d like, to obtain a copy of) what is called the Designated Record Set (DRS). This is all of the information held by OHSU that we use to make decisions about patients. The lists below are posted to help people understand what is included in the DRS. If you are ever unsure about whether a patient should be granted access to certain information in their file or chart, be sure to check with your supervisor for guidance.

Designated Record Set (these documents are only available on the OHSU Intranet):

A Patient’s Health Record, which includes:

  • Medical Records
  • Dental Records
  • Medical Billing Records
  • What is EXCLUDED from the Designated Record Set.
