Revised: Proposed Project Questionnaire (PPQ) form
The Proposed Project Questionnaire (PPQ) form that is required for all OHSU grant submissions has been updated to include a checkbox at the top left side for identifying applications and supplements in response to ARRA stimulus funding announcements.
All investigators should use the new PPQ starting immediately, whether you are applying for stimulus funds or not. You can access the form in Word and Adobe formats. http://www.ohsu.edu/research/rda/forms.shtml#ap
Reporting requirements on funds obtained as a result of the stimulus package will be extensive and must be tracked to ensure proper compliance. For PPQ questions, please contact your appropriate department admin or your Research Grants & Contracts analyst.
