The OHSU Department of Orthopaedics and Rehabilitation practice has three locations:

Monday through Friday, 8 -11:30 am and 1- 4:30 pm

To schedule, cancel, or change an appointment, call 503-494-6400.
  • Returning patients: Press #3; then press the designated number for your physician.
  • New patients: If you have never been seen in the Orthopaedics Practice or do not know the name of your physician, press #4.
  • If you do not have a physician or if you do not have a push-button telephone, please stay on the line to speak with a scheduler.
  • If you do not have medical insurance, call 503-494-1671 between 9 am and 4 pm on weekdays.
Appointments can be scheduled Monday-Thursday, 8:00 am - 5:00 pm, and Fridays 8:00 am - 4:00 pm.

Bring to your first appointment
  • your insurance card
  • all non-OHSU x-rays
  • CT scans, MRI scans, and bone scans
  • any other medical reports that describe your medical condition
Waiting time for appointments
Our providers hope to see patients within 15 minutes of scheduled appointment times. Please understand that emergencies may delay your appointment. Practice staff will notify you if your provider is delayed and may offer to schedule another appointment if the wait time is not acceptable to you.

Late arrival for an appointment: 503-494-6400
If you will be late, please call as soon as possible. We will attempt to see you as quickly as possible when you arrive. However, if your provider is unable to see you later than your scheduled appointment time, we will re-schedule you later that day, or we will schedule a new appointment date.

Canceling appointments: 503-494-6400
Please call at least 24 hours in advance to cancel your appointment. We know that emergencies arise and your plans may change. However, giving us as much notice as possible helps us better serve you and other patients.

If you miss two appointments without giving us 24-hour notice, we may be unable to continue providing your orthopaedic care.

After your appointment is scheduled, please call 503-494-8505 to register. Outside the Portland metro area, call toll-free 1-888-222-6478 x8505. You must register before your appointment if :
  • you are a new patient in the OHSU Orthopaedics Practice
  • you changed insurance since your last outpatient visit at OHSU
  • your OHSU registration card has expired
  • you were in an automobile accident or work-related accident and are being seen in the Orthopaedic Practice for the first time since the accident
If you are a member of an HMO or PPO, you must have an authorization from your primary care provider (PCP) or a visit approval from your insurance carrier for point of service care before you schedule an appointment in the clinic.

If you are enrolled in the Oregon Health Plan (OHP), you must choose an OHSU primary care provider before you make an appointment. Call 503-494-1671 between 9 am and 4 pm on week days for information.

Public transportation to OHSU
The Tri-Met No. 8 bus (Jackson Park) serves the OHSU campus. You can catch bus 8 in downtown Portland on the SW Fifth Avenue Transit Mall. Buses run about every 15 minutes. Call 503-238-RIDE or visit www.trimet.org for schedules.

Parking at OHSU
Our Practice is on the fourth floor of the Physicians' Pavilion. The entrance to the Physicians' Pavilion is on SW Sam Jackson Park Road (map of OHSU Physicians' Pavilion and parking). You will not be charged for parking during your appointment.
  • Short-term parking (4 hours or less): Park underneath the Physicians' Pavilion. Take an elevator from the parking garage to our Practice on the fourth floor, suite 430.
  • Long-term parking (longer than 4 hours): Continue past the Physicians' Pavilion on SW Sam Jackson Park Road. Turn right into Lot 13 across from the Emergency Department. Follow signs to "Patient Parking."
For valet parking, follow the "valet parking" signs or tell the parking attendant when you enter the parking garage that you'd like valet parking.

The OHSU campus has construction projects in progress. Please expect delays and allow extra time for parking. If you are unsure about parking options, please ask a parking attendant as you enter the Physicians' Pavilion parking area.

Insurance and co-payments
If your health plan requires a copayment or prepayment, please bring your payment to your appointment. We accept cash, personal checks, VISA debit cards, American Express, Discover, Mastercard, and VISA. We are happy to complete disability, insurance, and other forms for you; however, you may be charged a fee for these services.

Your bill includes amounts billed to your insurance and any amounts that are your responsibility. Please call if you have questions about your bill.

If you have an outstanding balance due, you may be asked to make a payment or pay the full balance at your next appointment.

Medical advice during practice hours: 503-494-8733
If you have medical questions, please call us. Medical advice is available from 8:00 am to 3:00 pm Monday-Friday, except on holidays.

We return most calls by the end of each workday. If we expect a delay in responding to your concerns, we will call to let you know when you can expect to hear from us.

Medical advice after hours: 503-494-8311
An orthopaedic physician is available by phone to talk with you about urgent medical needs after our regular office hours and on holidays. If you have an urgent orthopaedic need, call and ask the operator to page the on-call orthopaedist. The orthopaedist will return your call as soon as possible.

Prescription refills: 503-494-8731
  • For refills call and leave your pharmacy's name and phone number, name of prescription(s) that you need refilled, your name, and your telephone number.
  • Refills will be ready within 72 weekday hours after you call us. Some prescriptions require that we mail them to you or that you pick them up in the Orthopaedics Practice.
