- Research Biography:
Dr Englander’s research focuses on transitional care for uninsured and low-income publicly insured patients. She is currently the co-Principle Investigator and Medical Director of the Care Transitions Innovation (C-TraIn), a transitional care quality improvement program. C-TraIn is a complex, multi-component transitional care intervention that includes four main components including: 1) care from a transitional care nurse who sees patients in the hospital, does home visits, and helps coordinate care in the 30 days post-discharge; 2) inpatient pharmacy consultation and prescription medications at discharge from a low-cost value-based formulary; 3) medical home linkages whereby OHSU partners with and provides payment to three community clinics to provide primary care for uninsured patients who lack a usual source of care; and 4) monthly implementation team meetings that convene diverse healthcare stakeholders to integrate elements of the healthcare system and engage in ongoing quality improvement
In addition, Dr Englander is interested in risk prediction models for hospital readmissions, and in understanding social determinants that contribute to readmission risk.
- Academic Biography:
Dr Englander teaches residents and medical students on the OHSU medicine wards and at the Old Town Clinic. In Spring of 2009 and 2010, Dr Englander served as an Honorary Senior Lecturer at Mbarara University of Science and Technology (MUST) in Mbarara, Uganda, where she collaborated with students to develop ‘The Art of Medicine’ in East Africa, a curriculum that explores issues of provider-well being, professionalism, and healthcare ethics.
