Your privacy is important to the staff of the Student Health Service. We maintain a private record of your health care in the Health Service separate from your main health chart in the OHSU Medical Records department until your training ends. When you leave OHSU your student medical record will be transferred to the OHSU record (LCR).
There are three exceptions: First, any mental health records (specifically, any psychotherapy or counseling chart notes) contained in the student health service record will not be moved to the OHSU record. They will be kept permanently within the records of the student health service.
The second exception is that records of laboratory testing, X-rays, or clinic visits made at OHSU clinics outside of the Student Health Service will appear in the OHSU record.
Finally, the records of those specific immunizations required by OHSU as pre-requisites (based on CDC Advisory Committee on Immunization Practices for health care workers) are shared with your training administrators in the course of the immunization surveillance and reporting program, solely for the purpose of determining eligibility for clinical or research assignments.
If you have any concerns about confidentiality, a Student Health Service physician will review your records with you at any time.
