Intake ClinicThe intake clinic will be the first contact you will have with the OHSU Neuromuscular Disease Center. This first visit will allow the physicians to route you to the most appropriate follow-up clinic. Before you arrive... Transfer of Medical Records: It is important that we have any relevant medical records regarding your neurological condition prior to your appointment. We have several forms for you to fill out and send to your physicians. Please download (or have your physician download) and have your physician fill out the Intake form and the Referral form. If you are hand carrying your records, X-rays, or MRIs, please let our office know. Complete the NMD Intake form In addition to the forms which your physician needs to fill out, we ask that you fill out the new patient questionnaire (part of the OHSU Welcome Packet) and bring it with you to your first appointment. If you choose not to fill it out prior to your appointment, we will ask you to do so at the time of the appointment. Any Insurance preauthorization set up: It is advisable that you become familiar with your insurance plan. Knowing whether a REFERRAL or PRIOR AUTHORIZATION is required for your consultation or any additional testing will save time and unwanted bills. If you do not have health insurance, there are agencies which may be able to help you obtain some types of coverage. This can be done over the telephone prior to your appointment by calling (503) 494-8505 (or toll-free (888) 222-6478 extension 4-8505). You can register in person on the 3rd floor of the Physician’s Pavilion, just to the right of the elevators. Please arrive 30 minutes before your appointment if you choose to register in person. The day of your appointment... The day of your appointment be prepared to spend several hours in the clinic. This comprehensive initial clinic visit may include lab work and/or certain diagnostic procedures. In general an initial visit will consist of the following:
Should you have any questions or concerns during your visit to the Neuromuscular Disease Center please voice them to your physician or the clinic nurse. The goal of all our clinics, but especially the Intake clinic, is to educate you, the patient, and help you make informed choices about your medical care.
