Intake Clinic

The intake clinic will be the first contact you will have with the OHSU Neuromuscular Disease Center. This first visit will allow the physicians to route you to the most appropriate follow-up clinic.

It is important that we have any relevant medical records regarding your neurological condition prior to your appointment. We have several forms for you to fill out and send to your physicians. Please download (or have your physician download) and have your physician fill out the Intake form and the Referral form. Have these forms and your medical records sent or faxed to:

If you are hand carrying your records, X-rays, or MRIs, please let our office know.

In addition to the forms which your physician needs to fill out, we ask that you fill out the new patient questionnaire (part of the OHSU Welcome Packet) and bring it with you to your first appointment. If you choose not to fill it out prior to your appointment, we will ask you to do so at the time of the appointment.

It is advisable that you become familiar with your insurance plan. Knowing whether a REFERRAL or PRIOR AUTHORIZATION is required for your consultation or any additional testing will save time and unwanted bills. If you have never been a patient at OHSU before, or if your medical card has expired, you will need to pre-register. This can be done over the telephone prior to your appointment by calling (503) 494-8505 (or toll-free (888) 222-6478 extension 4-8505). You can register in person on the 3rd floor of the Physician’s Pavilion, just to the right of the elevators. Please arrive 30 minutes before your appointment if you choose to register in person.

The day of your appointment be prepared to spend several hours in the clinic. This comprehensive initial clinic visit may include lab work and/or certain diagnostic procedures.

Should you have any questions or concerns during your visit to the Neuromuscular Disease Center please voice them to your physician or the clinic nurse. The goal of all our clinics, but especially the Intake clinic, is to educate you, the patient, and help you make informed choices about your medical care.

In general an initial visit will consist of the following: Nurse Assessment: The Clinic nurse will get basic information about your symptoms and overall health, as well as your vital signs. Physician Assessment: The Neuromuscular physician will review your symptoms, and find out about your medical history Exam: Next the Neuromuscular physician will conduct a thorough exam. This may include measures of muscle strength. Diagnostic plan-labs, diagnostic procedures: Depending on what the Neuromuscular physician finds, they may want to order further tests. These may be blood tests, or other specialized neuromuscular tests like an EMG, a muscle biopsy or nerve biopsy. Treatment options/plan: Following the exam, the Neuromuscular physician will discuss their findings with you, and suggest possible treatments. Follow-up plan: In consultation with you the physician will outline a plan for your follow-up (if any is needed) in the Neuromuscular Disease Clinic.
